Patient Services

Insurance Participation:

We participate with most medical insurance plans, including Medicare, Michigan Medicaid, Blue Cross Blue Shield, BCN, Aetna, Cigna and HAP. If you are referred by an HMO, you will need to bring the appropriate authorization form with you. Payment of service charges, uncovered testing, co-pays and deductibles are expected at the time of your examination. Please see our Financial Policy for more detailed information.

We do not participate in medical discount card programs or vision insurance (glasses) programs.

If we do not participate with your insurance, you will be given a receipt and the necessary insurance codes to submit for reimbursement. Payment, however, is expected at the time of your examination.

We will make every effort to help you submit your claim, however, payment is expected at the time of your examination. If you have any questions concerning your coverage we are happy to assist you prior to your examination.

If you have any questions concerning your coverage we recommend that you discuss them when you make your appointment. It is often difficult to tell you in advance if special tests or studies are going to be necessary since these are dependent on the nature of the problem and our findings during your visit.
