Eye Emergencies //

If you are one of our established patients, one of our doctors is always available to speak with you if you feel that you are having a serious and worsening problem related to the cornea condition for which you see us. If such is the case, and if the office is closed, please contact our answering service at 248-544-6931 and they will get in touch with the doctor on-call. For routine questions and matters, please contact us during normal business hours (M-F 8AM - 4:30PM).

Paying your Bill and Questions About Your Bill //

If you would like to pay your bill, please call our Southfield office at 248-350-1130.  We accept all major credit cards over the phone.

 

If you have any questions about your bill, please contact our billing provider, Professional Billing Services, by calling them at 734-207-1690.

Release of Medical Records //

Below is a form to be filled out to allow your medical records to be released to our physicians.

Insurance Participation //

We participate with most medical insurance plans, including but not limited to:

 

Medicare

Michigan Medicaid

Blue Cross Blue Shield

BCN

Aetna

Cigna

HAP

 

If you are referred by an HMO, you will need to bring the appropriate authorization form with you. Payment of service charges, uncovered testing, co-pays and deductibles are expected at the time of your examination.

 

We do not participate in medical discount card programs or vision insurance (glasses) programs.

 

If we do not participate with your insurance, you will be given a receipt that you may submit for reimbursement. We will make every effort to help you submit your claim, however, payment is expected at the time of your examination. If you have any questions concerning your coverage we are happy to assist you prior to your examination.

 

If you have any questions concerning your coverage we recommend that you discuss them when you make your appointment. It is often difficult to tell you in advance if special tests or studies are going to be recommended since these are dependent on the nature of the problem.

With the closure of Michigan Cornea Consultants, please click on the button below to see options for obtaining a copy of your records.  Please note that if you plan to continue seeing Dr. Dunn, Dr. Heidemann or Dr. Chow, they already have access to your records and no special action on your part is needed.

Accessing Your Chart (Patient Portal) //

The patient portal allows you to access your medical record and to enter your own Pharmacy information, Past Medical History, Ocular (Eye) Disease History, Medications and Allergies History.
